Jet Medical Center® is pleased to offer immigration physical services. Dr. Choksi is a licensed US Civil Surgeon who has been authorized by USCIS to conduct Immigration Physicals for the Department of Homeland Security. Individuals applying for citizenship or seeking permanent residency or a Green Card for employment are required to obtain an immigration physical performed by a designated Civil Surgeon prior to approval of their application. The exam results will be recorded on Form I-693 for the applicant to return to the USCIS (INS). The exam process generally takes 1 week, depending on lab processing times. Our goal is to efficiently and affordably provide immigration physical exam evaluations with appropriate testing in the shortest time. We welcome you to the United States to live the American Dream!

N-648: MEDICAL CERTIFICATION FOR DISABILITY EXEMPTIONS
Having trouble learning English or studying for the civics exam due to a medical condition? We’re here to help! Our team carefully reviews your medical history and works with your specialists to determine if you qualify for a medical exemption. We take extra time to properly document and explain to USCIS why you may be unable to learn or demonstrate knowledge of English and/or U.S. history and civics due to a physical, developmental, or mental condition. Many basic evaluations rely solely on stating that English is not the patient’s first language; however, we have found that this alone is insufficient. Instead, we provide thorough, accurate medical documentation that meets USCIS requirements and helps support your case. Please call us for more information.
